In Fiscal Year 2023-2024, Harbor Regional Center identified priorities for resource development. The Department of Developmental Services approved 2023-2024 Community Placement Plan (CPP) and Community Resource Development Plan (CRDP) Funding for these services.
We are pleased to share that the following service providers were awarded these CPP/CRDP Grants.
(CPP) The development of an ARFPSHN that will provide 24-hour medical/nursing care and intensive supports for individuals transitioning from various skilled nursing facilities who may have various specialized healthcare needs including but not limited to: insulin dependency, tracheostomy, renal dialysis, wound care, etc. A Housing Development Organization (HDO) owns the home and the provider will lease the property from the HDO. The provider is responsible for activities integral to the establishment of the licensed home, e.g. licensing, household furnishings and supplies, personnel and consultant recruitment, staff development and training, transition planning, meetings, travel related to resident visits, etc.
(CRDP) The development of a partial community inclusion day program to support individuals with Intellectual and Developmental Disabilities who want to remain active in their community through a variety of daily activities and who choose to work less than full-time. Participants will require increased positive behavior supports around their activities of daily living, communication, social engagement, health and independence. This program will support individuals who are interested in employment through the developmental of micro-enterprises and other competitive employment options in their community. Activities will occur both on the site and out in the surrounding community. The partial community inclusion model is based on participants being engaged, on average, in their community for 1/3rd of the service time. Activities off-site should occur in small groups of not more than three participants.

The development of a center-based Infant Development Program capable of providing services for up to 40 – 50 children that is located in or around the cities of Wilmington, Lomita, Harbor City or San Pedro in Harbor Regional Center’s catchment area. This will be a multi-disciplinary program serving infants and toddlers from the age of approximately 18 months up to 36 months who have been found eligible for early start services. The Infant Development Program shall provide intervention activities which promote development in the following areas according to the basic principles of child growth and development: physical, cognitive, language and speech, psychosocial, and self-help/feeding. The program shall utilize developmentally appropriate practices ensuring interactions are based on children’s individualized needs. The program shall implement and practice the foundational principles, competencies, and evidence-based practices. The program shall develop and integrate the expected level of participation and attendance by parents and/or families/caregivers, including approaches that are respective of language preferences and cultural beliefs and practices.
(CPP) The development of a level 4I licensed Adult Residential Facility (ARF) home for up to four (4) adult individuals who qualify to receive regional center services. The primary focus of this home will be to provide a safe living environment for individuals requiring enhanced support due to involvement in the criminal justice system who also have significant behavioral support needs and/or a co-occurring mental health diagnosis. Individuals with intellectual and developmental disabilities (IDD) have the same right to justice and fair treatment in all areas of the criminal justice system as those without disabilities and therefore must be afforded the support and accommodations required to ensure fair and equal treatment. Along with meeting the requirements to open and manage a licensed residential home, the applicant must also have the relevant experience, understanding, and knowledge of how to support individuals with IDD who are involved in the criminal justice system. While the circumstances and needs for each individual residing in the home will vary, examples may include but are not limited to: being actively involved in the court system, being on a diversion program/plan, and/or are going through competency training.

The development of an Enhanced Behavioral Support Home (EBSH) is intended to serve a maximum of four (4) adults who require 24-hour non-medical care in a homelike setting due to excessive behavioral impairments. This home exceeds the minimum requirements of level 4I homes and shall employ enhanced characteristics, such as delayed egress on the perimeter exits, to address challenging behaviors for individuals who require increased behavioral supports staffing, direct supervision, staff training and certification. The individuals in the home will have increased monitoring by regional center case managers, behavioral professionals, quality assurance staff, including employees of the Department of Developmental Services (DDS).
Acquisition of one single-family home through a qualified Housing Development Organization (HDO) and subsequent renovations for the purpose of developing one Adult Residential Facility for Persons with Special Health Care Needs (ARFPSHN).
The development of a community-based partial inclusion day program to support individuals with Intellectual and Developmental Disabilities that want to remain active in their community through a variety of daily activities who choose to work less than full-time. Participants will require increased supports around their activities of daily living, including those with complex physical and/or medical conditions. This program will support individuals who are interested in employment through the developmental of micro-enterprises. Activities will occur both on the site and out in the surrounding community. The site will be located in Norwalk, Bellflower, Cerritos, or Artesia in the Harbor Regional Center catchment area.
The development of Behavior Respite Services to provide temporary relief to family members and caregivers from the responsibility of caring for children and/or adults with intellectual and developmental disabilities (I/DD) who also demonstrate challenging behaviors.
The development of a Financial Management Service (FMS) business to help individuals enrolled in the Self-Determination Program (SDP), Participant Directed Services, and Supported Employment program(s) pay for services they need.
The development of a Family Home Agency (FHA). A Family Home Agency (FHA) approves family homes which offer the opportunity for up to two adult individuals, with developmental disabilities, per home with individual bedrooms to reside with a family and share in the interaction and responsibilities of being part of a family. The individual(s) with developmental disabilities receive the necessary service and supports from the family, agencies, and the community to enable them to be a participating member of the family and the community where the family resides. FHAs are responsible for recruiting, training, approving and monitoring family homes, as well as providing ongoing support to family homes. Social service staff employed by the FHA make regular visits to the family home to ensure that necessary services and supports are in place, and that the match between the family and the new family member is viable, and continues to be viable.

The Housing Development Organization (HDO) will acquire and renovate two (2) single-family homes within Harbor’s catchment area for the development of permanent housing, licensed and certified as Enhanced Behavior Support Homes, for up to (four) 4 individuals in each home.
Development of a START (Systematic, Therapeutic, Assessment, Resources, and Treatment) program/team that will implement the START model which provides lifespan (ages 6+) crisis support services for individuals who have co-occurring Intellectual and Developmental (IDD) and mental health support needs. The local START Team will provide prevention and intervention services through crisis planning and response, education, consultation, and coaching. The goal of the START program is to improve diagnosis and treatment, support effective services, promote health and wellness, decrease the need for emergency services, and build capacity through relationships across service systems to help people remain in their homes and communities while enhancing the ability of the community best support them.
In 2020-2021, we identified priorities for resource development. The Department of Developmental Services approved 2021-2022 Community Resource Development Funding for these services.
The following service providers were awarded these Community Placement Plan/Community Resource Development Program project grants.
The Early Intervention Site based program will serve up to 50 children from ages 18 months-3 years in the North Eastern part of the Harbor Service Area. This program will provide an infant development program to Early Start children identified with moderate to severe developmental delays. Staff will have language capacity to serve Spanish-speaking families.
The Dental Project will serve Harbor Regional Center children and adults who are without adequate access to dental health resources and may benefit from a community-based resource. The project will use funding to collaborate with a local health clinic or dental group to provide dental sensitivity training, non-anesthesia dental services, and periodontal maintenance for individuals served by Harbor Regional Center.
The Specialized Supported Living Services will serve 15 individuals transitioning from Porterville Secure Treatment Program or requiring a community living alternative for deflection due to involvement with the criminal justice system and/or affected by repeated psychiatric hospitalizations. Startup will include extensive transition services and specialized training for staff to support individuals who may have co-occurring mental health issues or present with serious behavioral challenges that jeopardize living arrangements and can result in homelessness.
During the ongoing pandemic, it has become evident that a portion of individuals and families services by the Los Angeles County Regional Centers have been unable to access services and supports due to not having access to appropriate technology. The Technology Lending Library will increase access to remote and virtual services for individuals and families by providing eligible participants with tablets and/or laptops, data service, and technical support so that they can better access remote services as providers innovate alternative and remote ways to deliver services. The contracted provider will purchase hardware, software, and data service, as well as provide technical support, including adapting equipment that may be needed by some individuals.
The START Model is a research-based model of community-based crisis prevention & intervention services for individuals 6 and older with intellectual and developmental disabilities and mental health needs. The Center for START Services (CSS) at the University of New Hampshire Institute on Disability/UCED is an organization of experts in the mental health aspects of intellectual and developmental disabilities who develop innovative MHIDD training programs, conduct research, and facilitate the development of START programs across the country.

The following service providers were awarded these Community Placement Plan/Community Resource Development Program project grants.
Harbor Regional Center, in collaboration with Regional Center of Orange County and the Southern California Integrated Health and Living Project (SCHILP), obtained proposals for the development of specialized resources for individuals with intellectual and developmental disabilities with co-occurring mental health conditions, who resided in locked and restrictive settings including, but not limited to, IMDs, Out-of-State facilities, or at risk of placement into an institutional setting.
Harbor Regional Center requested proposals for the development of a specialized Supported Living Service to serve persons with a variety of mental health, behavioral or health challenges, including individuals that are transitioning from Porterville Developmental Center Secure Treatment Program, or those requiring a community living alternative for deflection due to involvement with the criminal justice system, and/or affected by repeated psychiatric hospitalizations.
Harbor Regional Center obtained proposals for the development of an Intensive Employment Services program for individuals who exhibit significant behavioral challenges and/or co-occurring mental illness. These Individuals are transitioning from Porterville Developmental Center Secure Treatment Program, are court involved, or are at risk for forensic involvement. Emphasis shall be placed on employment services and life skills development.
Crisis Intervention Services $225,000

Development of a Crisis Intervention Service for people who are at risk of being moved from their family home, or other living environment into a more restrictive setting. The service will support and stabilize individuals who are in crisis in order to maintain their current living situation and prevent moving to a more restrictive setting.
