Service Provider Directory

On October 29, 2024, DDS  launched the Provider Directory. Participation in the DDS Provider Directory is mandatory for all providers that meet the criteria. Providers that do not submit the required information in the Provider Directory by May 30, 2025, risk having their billing suspended beginning July 2025. For service providers included in Rate Reform, participation is the only way to attain or keep the 10% quality incentive portion of the new rate.

If you believe that you already submitted your record(s), we recommend you login to the Provider Directory and verify that no records are awaiting your action.
