What documentation do I need to submit to the regional center when requesting vendorization?

In addition to a Vendor Application (Form DS 1890), and the Applicant/Vendor Disclosure Statement (Form DS 1891), applicants must submit the documentation specified in Title 17, Section 54310. If approved to move forward in development with Harbor, the Vendor Coordinator will provide the required vendorization packet.
