Each year Harbor Regional Center Counselors conduct an Individual/Family Service planning meeting, to identify our client's individual strengths, needs, and desired outcomes, and to develop a plan for the year to come.

We also use this opportunity to collect other important information which may help us to serve our clients better. For example, we ask our clients or their families to fill out and mail in the Individual/Family Survey.

In this survey we want to learn what type of family support you may be interested in: parent-to-parent support, parent support groups, or client self-advocacy groups. We also suggest various ways that our parents and clients can become active participants at Harbor Regional Center. The survey also gives clients and families the opportunity to express their preferences regarding their assigned service coordinator.

This survey is available for HRC Clients and Families to complete on line.
