Harbor Regional Center works with each individual client or family to develop an individualized plan. This is called the Individual/Family Service Plan. The plan describes the individual’s preferred future in terms of outcomes that the person and his family wish to achieve.

It also describes how to get to those outcomes, in terms of the services and supports needed to achieve them.

Harbor Regional Center then assists our clients and families to obtain needed services which have been identified in the individualized plan.

Individual/Family Survey
In this survey we want to learn what type of family support you may be interested in: parent-to-parent support, parent support groups, or client self-advocacy groups. We also suggest various ways that our parents and clients can become active participants at Harbor Regional Center. The survey also gives clients and families the opportunity to express their preferences regarding their assigned service coordinator.
